My COVID Record

My COVID Record allows you to  

  • check your vaccination record is accurate and up to date 
  • request an official record of your COVID-19 vaccination status – the My Vaccine Pass for use in Aotearoa New Zealand, or a separate International Travel Vaccination Certificate for use overseas. 

How to sign up for My COVID Record  

You will need your own email address to create an account. 

Once you are on the mycovidrecord.health.nz you will be asked to sign up to a My Health Account, so you can access your Covid Record online.  To do this you will need to create a password, confirm your email address, and prove your identity. 

If you don’t have one of the following forms of identity, you can ring the My Covid Record helpline 0800 222 478 (available 8am to 5pm, Monday to Friday). 

A New Zealand  

  • driver licence 
  • passport 
  • birth certificate 
  • or citizenship certificate. 

An Australian: 

  • passport 
  • birth certificate.

What if I don’t have one of the six forms of ID needed to sign up for My Covid Record?  

If someone has been vaccinated, but doesn’t have an ID they can call 0800 222 478. We suggest that they have their National Health Index (NHI) number ready, but if they don’t have it we may ask a few questions to confirm your identity – such as your name and date of birth.
